Students are required to have proof of TDap, MMR, Varicella, HepB, Meningococcal,
& TB immunizations. A healthcare provider needs to complete and sign the immunization
form (located here: Immunization Form) and Physical Examination Form (located here: Physical Exam Form).
You will also need to provide your immunization records. You will need to upload both
your immunization form and immunization records to the CastleBranch software system. After you submit the Registration form our staff will contact you
and provide you with your Student I.D. Your Student I.D. will also serve as your Pin
Number. You will need your Pin Number to upload your immunization form and medical
records to CastleBranch.
